Voice recognition software combined with VOCA

Hi. I am a grad student finishing an Assistive Technology program, and
also teach teenage students with multiple disabilities. I have used voice
recognition software (VR), but need to now combine it for several purposes.
If anyone has suggestions, I would appreciate hearing them. Here is a brief
description of the student I'm targeting a good AT "fit" of devices:

Female 14 years of age with low vision, learning disabilities, ADHD & OCD. She
reads with comprehension on a first grade second semester level, but
understands information presented verbally on a much higher level--nearly a
4th grade level. Her writing level is very depressed: about a first grade
level at best. Her reading/writing ability are not in line with her
potential.

My best guess is for a scanner to scan in all text, have voice
recognition software, voice output software, and screen enlarged software.
    
Any suggestions would be greatly appreciated!
